1.Horizontal chop

日本の術者のビデオをみると、horizontal chopを行っている場合が多い。僕もhorizontal派だ。

2.Vertical Chop

海外の術者はvertical chopを採用している人も多い。

3.Horizontal vs Vertical

Horizontal :Additionally, the very end of the tip must be dull, or even oliveshaped, to protect the capsular bag.

Vertical:in contrast to the dull distal end of a horizontal chopper, the end of a vertical chopper must be sharp to penetrate the nucleus easily.

horizontalの方は先が丸いものを使うことが、vertical chopperは先が鋭利なものを使う。これは手法の違いを見れば明らかだ。

2024/10/13追記

Vertical Chopは核がある程度やわらかくないと不可。ので、Horizontalの方が汎用性がある。手法が普及しているのには理由があるってこと。

当院でアイハンスでミニモノビジョンをしてますが、これを参天のレンティスコンフォートで行うというお話でした。

どのような患者さんを適応とするかについては

1.患者さんの意向

2.多焦点にしたいが、費用を気にしている場合。

ということで当院と同じ。

ターゲットは-０．５-から-1.0Dですが、FEST法のような度数同定はしていない感じでした。

広田眼科の廣田篤先生によると、

トーリックレンズは

直乱視１．５D以上ならいれる。
倒乱視０.７５D以上ならいれる

当院は-0.75D以上なら、直、倒にかかわらず挿入しています。
